Most would probably indicate either of these for anxiety after more conventional treatments have failed.
I would tend to put them earlier in the trial process though. If nothing else, I would want to compare them briefly (a week or two each) with whatever other drugs I was trying just to make sure I was on the best match for my chemistry. After quick comparisons, then choose the best for a longer term trial. Anxiety usually responds very quickly to the right medication (within days, often the first day), so lengthy trials are an unneeded torturous process. If a drug isn't helping in the first week, the odds of it helping with continued use are not encouraging. It could indeed work great with continued use through a trickle-down process, but I prefer to be on a drug that targets the chemistry directly because quick improvement is the result. Most doctors would be argumentative with my opinion, but they haven't personally tried dozens of these drugs like I have.
In my experience, the closer a drug is to targeting the problem, the quicker the results. And I would definitely put Zyprexa and one or two other antipsychotics near the top of the list for quick comparisons just to see. I want to be sure I am on the best match and not leave any stone unturned.
