GEODON have an antidepressant effect?

Posted by elderweissblue on June 15, 2001, at 19:20:19

Iam on zyprexa 5mg but wished it would do something for my sometimes depression which can be occasionally bad. (I had a bad reaction on prozac- agitation). Klonopin sometimes help with depresssion and sometimes the opposite. sometimes I think zyprexa might have added to depression (this was before I went on klonopin). I wonder if I should switch to Geodon, some people say that it has a antidepressant effect. Is the serotoningistic reuptake stronger than in zyprexa? Any answers.

Zyprexa had no antidepressant qualities for me, but Geodon does, apparently. I take 160 mg in divided doses. It started to work in 9 days for my unipolar non-psychotic depression.

Ziprasidone (Geodon) has not been studied specifically in the treatment of depression. However, its pharmacological make-up makes it a potential antidepressant: 1) reuptake inhibition of norepinephrine and serotonin, similar to some classes of antidepressants, and 2) antagonism of serotonin receptor 1D (5HT1D). During the pivotal antipsychotic trials of Geodon in schizophrenia and schizoaffective disorder patients, both dosages 120 mg/day and 160 mg/day have been found to significantly improve depression and anxiety symptoms as compared to placebo.

Olanzapine (Zyprexa) by itself is not an adequate antidepressant. It is commonly used as an augmenter to the antidepressants in the treatment of partial or treatment-resistant depression. In one clinical study, it was found that the combination of fluoxetine (Prozac) and Zyprexa was found effective in treatment-resistant depression. A couple of caveats about this study: 1) it involved a small number of participants, 2) it was funded by Eli Lilly, the maker of both Prozac and Zyprexa, and 3) at least one of the clinical investigators is known to be on the payroll of Lilly.

Is the serotonergic reuptake stronger (in Geodon) than in Zyprexa?

Actually, of the currently available (US) atypical antipsychotics, Geodon is the only one that possesses the unique pharmacological action of reuptake inhibition of norepinephrine and serotonin.

FWIW, the following compares the antipsychotics' degree of "atypicality" based on the ratio of 5-HT2A to D2 receptor binding affinity:

Ziprasidone (Geodon) 11

Risperidone (Risperdal) 6.9

Clozapine (Clozaril) 5.5

Olanzapine (Zyprexa) 2.7

Quetiapine (Seroquel) 2.2

Chlorpromazine (Thorazine) 0.3

Haloperidol (Haldol) 0.016

If you decide to switch from Zyprexa to Geodon, it should be transitioned gradually. You may experience insomnia/anxiety if the done abruptly.
