Hi everyone.
I finally got a prescription for nardil today. PDoc wasn't too keen and tried to convince me that TCAs were more effective, which I didn't exactly agree with, especially seeing as im treating SA.
But anyway, I was a little bit disapointed at the amount I got. 2x15mg a day... seems a little bit small don't you think? Considering the BNF guidelines recommend starting at 45mg... Do you think 30mg is going to be any help for social anxiety? I have another 3 weeks before my next appointment, not sure how much he'd be willing to increase the dosage.
This whole thing is so slow, its one setback after another.

Right I guess I'm just impatient. Interesting that you felt an effect at only 15mg, I thought that it was more of a plateau thing where you got the effects all at once rather than gradually.

Hi J,
If you start with a low dose you can always increase... but if you start with a dose which is too high you might have side effects which could put you off the drug for life!
Like you.. I am impatient!
I look forward to hearing how you do on Nardil.

My pdoc has over 20 years of experience with Nardil, and I believe the dosing regimen he had me follow is pretty close to what's in the PDR. I started on 3 x 15 mg/day for a week or two to see if I could tolerate it, then met with him again before going up to 4 x 15 mg/day, which seems to be considered more or less the minimum effective dose. In my case, the 45 mg/day had no adverse or therapeutic effects, 60 mg/day had mainly adverse effects (insomnia), and it wasn't until I got up to 75 mg/day that I got a noticeable antidepressant effect.
As I've mentioned many times before, there is one school of thought on Nardil that a person needs to get to 1 mg/kg body mass/day to be sure that they are giving the drug a fair trial. In my case, at about 68 kg (150#), this did correlate, as I did not notice substantial therapeutic effects until I got to 75 mg/day. As I said, it is just one school of thought on the drug and may or may not stand up to rigorous analysis; my pdoc said he had not actually heard of it.

I'm for titrating slowly, too. I did very well at 45 mg and then 60mg. Based on body weight (I'm ashamed to admit) I should need 75mg. I could not tolerate that dose. It had me jumping out of my skin with severe restlessness and insomnia. 67.5 mg was much better. I am currently at 52.5 mg since I am trying to get the best benefit while keeping side effects (edema and weight gain) low.
