Doxepin (Cinequan) anxiety at the beginning?

Posted by xtof on November 29, 2008, at 9:52:32

I have been taken Remeron for some weeks at low dose, but eventually had to come off it bcs I couldn't sleep of it and it gave me anxious dreams. So I switched to Doxepin for 3 days now (at 25mg at night). It helps me to sleep a bit better, falling asleep is still a problem though. But the main thing that worries me is that my anxiety level is way up again :( Is this a normal starting up reaction and should I hold on to it for some period?
I also take 35mg of Amythriptiline for aver a year now but I don't notice any effect from that anymore. I once went upto 75 of Ami but was sweating thru the night. And my mood didn't get any brighter with it. So now I'm still suffering from this anxiety/depression feelings. So any advise on the Doxepin? I know that AD effects can last some weeks before it hits, but isn't the anxiolictic effect immediately noticeble?
I guess I should also mention that I have taken testosteron gel for 3 weeks (but quit now), maybe this can have caused an increase in my anxiety level?

I have been doubting to try Buspar but it scares me off. I take a benzo daily (at least one Ativan)and that has always been a big help, both to the anxiety as mood uplifter. Then adding buspar or taking buspar instead of ami or doxepin might be worth a shot?

Sinequan can be used for sleep so I think I was taking 10 mg. But it is weaker in that department than amitriptyline so I got used to it quickly.

Tha actual PI noted a starting dose of 75mg but I would not go there. 25mg sounds reasonable.

Not really listed in the PI anything about anxiety symptoms, are you sure its anxiety or not a rather more rare EPS symptoms such as akathisia (there is some blockade at D2 with TCAs but its pretty low)?

Or it could just be endogenous anxiety that you already have ?

Most people who havent been exposed to TCAs or any type of drowsy antihistamine for very long experience lethargy and sleepiness at the beginning. That's the typical effect.

But since they and certain psychotropics have anticholinergic effects, anything is possible, nervousness I think is one, chills (I got that reaction), blurry vision, pulse changes, etc, just like any belladonna related medication, e.g. cogentin (granted not as powerful as belladonna itself which is no longer used directly, at any rate).

Some of these will probably go away as things go on for a few weeks but if the anxiety continues, I'd discuss it with your doctor. There are other TCAs, if thats the route you want to go.
