I decided to raise my Nardil dose to 90mg this morning...the energy/concentration/motivation part of the med had died at 75mg (I was on 75 for months). I'm assuming this is a DA issue.
I was originally going to add Memantine before raising to see if I could get that part of Nardil back but raised instead...now I have this Memantine and not sure what to do with it.
So...question. Should Memantine be added to Nardil while it is still effective (ie sometime in near few weeks as 90mg stabilizes) or wait until 90mg "poops out" then add?
Augment to maintain effectiveness or bring back effectiveness?

Just guesses.
I would add it now. You cover both scenarios with having the drug on board now. It is harder to recapture an antidepressant response once it is lost than it is to maintain it.
A friend of mine told me that she didn't feel the positive effects of memantine until she was on it for at least a month.

She remained absolutely healthy for over eight years. Then she changed doctors. The new doctor decided that his first intervention would be to have her discontinue the lithium. She relapsed into depression three weeks later, whereupon lithium was restarted at the same dosage. Nothing. The therapeutic response was not recaptured. She is going on 20 years of unrelenting double depression despite being a research patient at the NIMH for over 1 1/2 years and trying just about everything out there.
My own story reflects hers. I once responded with a 100% improvement - total remission - using a combination of Parnate and desipramine. It never pooped out. However, my doctor at the time had me discontinue it. When I relapsed two months later, he gave me Parnate only. For months I languished while I let him play with dosages. Then, when desipramine was again combined with Parnate - nothing. I could not recapture the therapeutic response.

...and I was doing in retrospect pretty A-OK on a combination of 225 Effexor + 200mg Lamictal 4 years ago. I decided to slowly taper off both, figuring "oh heck i'm feeling ok, i don't need these anymore".
relapsed a few months later.
now going on 4 years of trials...across the board, through all classes of meds, dosages, and combos, and rTMS, and ECT....so far, nothing has taken me back to where I was when I was stabilized on 225 Effx + 200 Lamict.
So there's another datapoint for you.

> - Scotti don't know if that still isn't the same thing. i don't think taht ADs ever do more than say 20% of the anti-depressing, but sometimes the stars line up right so that that 20% is enough for someone. at least, thats my interpretation of the placebo effect data.
