What is the role for anticholinergics such as Spiriva in chronic lung disease?

Sorry that I don't know what the "rule" is but ... I use Spiriva every day per my pulmonologist. It's usually prescribed (once a day) for COPD. I have the latter (mildly) even though I've never smoked. As far as I can tell I haven't had any side effects because it seems to stay in the lungs. Usually, if there were side effects to be had, then I would have them! <grin> It's probably used in other lung diseases to relax the smooth muscles (my guess) but you'd have to ask someone else about that.
