i have tremendous difficulty dealing with authority/medical figures, which is ironically interfering with the quality of my treatment.
given i have spent a lifetime avoiding confrontations, my guess is there isn't gonna be a quick fix - and that this is probably something best dealt with by a therapist - but are there any good books on assertiveness/confrontations/extinguishing passive-aggressive traits i could use to get the ball rolling?

I'm not all that much a fan of assertiveness and confrontation. I am, on the other hand, a huge fan of pragmatism. Marsha Linehan's book talks a lot about "doing what works", which is really the same thing.
It's not focused solely on assertiveness, but it includes for asking for what you need and letting people know how their actions and words affect you. And just as important, it also includes deciding whether or not any given situation is a good time for practicing assertiveness or if another skill might be better suited.
Don't let the name fool you. The skills taught in this manual are ones that can be of benefit in a very broad range of issues.

I don't know if you are male or female, but there are a number of good books written specifically for women. I like Harriet Lerner's "The Dance of Anger" or Nanette Gartrell's "My Answer is No . . . If That's Okay with You: How Women Can Say No with Confidence."
