Out on the web to see what Promethazine is: better know as Phenergan, an Antihistamine, of the phenothiazine-type used as an Antiemetic.
Phenothiazines are anti-psychotics, so they probebly effect 5ht. Also it is interesting that

"Promethazine is a phenothiazine derivative. Although the likelihood of those side effects associated with antipsychotic phenothiazines occurring with promethazine seem to be minimal, the possibility exists, especially at higher doses or with prolonged administration."
So it is possible that this is Serotonin-syndrome, NMS, or tardive dyskinesia. Please don't go running to the Doc to blame him/her for causing this. These conditions are uncommon, and some seem more sensitive than others. Also we don't really know all the how's and why's and how much issues. I am also way out of my area of knowlage as I have not taken Li or anti-psychotics b4.
It is more important to make sure this person is seeing a doc who has treated these conditions b4.
Is this person receiving any treatment for this new condition ?james
