Welcome the the world of HMO's. The practice of excluding meds is called formulary. An HMO has a list of formulary meds it will pay for. HMO's have done this for some time. Effexor, remeron, and Dexadrine are all non formulary for me. My doc can request approval of non formulary meds but this process usually leaves me weeks w/o meds.
