You already have all of my suggestions for what to talk to him about. I have one more pearl of wisdom for you (ha-ha, I crack me up). You are not wasting his time! You are paying for his services and you have every right to expect nothing but his best. Let him know (nicely) that you expect to be listened to. You may be able to tell that I've been burned a few times by doctors, and I have little tolerance for those who don't take our best interests to heart. Sounds like you've been doing your homework and I think you'll be well prepared when you get there.If you'll pay my airfair I'll go with you. I've always wanted to see Canada.....
