Did you find that the Effexor helped? Have you ever checked out a pysiotherapist?
Try keeping a "journal" for the headaches. After awhile you may find a pattern. I personally don't find that Effexor helps for pain. Other ADs have been more effective, but like you mentioned they have side-effects.
What are you using now? I find baclofen helps. I don't need to take it continuously, I can skip some days and once the pain starts coming back I start again.
