I don't know what Dr. Kramer will recommend, but I'll offer a suggestion .
Have you ever tried TCAs like Elavil or Doxepin or Pamelor?
My wife had terrible IBS, which is controlled by Elavil. One of her main symptoms was waking up with stomach pain, nausea, and vomiting.

Sometimes even a very low dose such as 10mg or 25 mg of a TCA like Elavil can really help, and TCAs get forgotten because of all the glamorous new meds. For some people, they work wonders.
