Your absence of response to Neurontin and Effexor leads me to recommend Neurontin's Cousin: Lamictal (same basic action), and Effexor's rival: Remeron. Combos are also good. The Effexor + Remeron combo is called "big gun" therapy by Dr. Stahl, and is comparable to ECT for depression.
