HI yes sympathetic block is from norepinephrine being replaced by a so-called false neurotrasmiiter ""octopamine"", hence octopamine is released from norepinephrine secretory granules--""storage vesicles"" , but octopamine has virtually no effect, it's only 1% potency of actual norepinephrine in it's abilty to provoke a post synaptic response ,thus a functional(((sympathetic block))) is manifested. Yes this indeed is the main culprit causing possible lethargy- as dysautonomic effects from the block , impede normal cardiovascular responses--such as inc. in cardiac output to exercise, and of course the low blood pressure in the standing position causes a sense of weakness. .............................................................................This sympathetic block becomes most intense in early morning hr's. as false neurotransmitter is allowed to build during sleep, but during waking status little by little it is displaced and some degree of sympathetic function is regained. Like I mentioned, this sympathetic block seems to be real troublesome for some , others barely notice it- depends on just how much of norepinephrine gets replaced by the ineffective "octopamine" , it never totally displaces--""replaces"" all the norepinephrine just a %, so some norepinephrine is released with the octopamine!!!!!!!!!!
