Absolutely yes, it can cause acne. It broke me out like hell.
I didn't know it was the fish oil. I had been taking it sporadically. All I took was two capsules a day. That's about 40% of what people here say to take.
I started taking it the second time. This time I took it religiously for a month. Finally, I figured out the acne had to be from it.
I quit and waited until the effects were gone. I started taking it again. This time, it was only day two and I broke out. I took it two more days and regretted it. I break out like my teen years with that stuff. I'm sorry, because it had a good effect on energy.
I bought flax oil and seemed to be having more problems again. I'm not sure yet. I stopped taking it. When I know nothing else will change, I'll start taking it again so I can be sure. I also got CLA (another oil) to help with weight loss after reading some posts here and I'm afraid to take those capsules, so I haven't yet.

Yes, it is because you rest your face on your hands. It's the oils from your body. My dermatologist told me that it wasn't what got in my body, but what I got on my face that caused the problem. My question to him was about foods. Obviously, he didn't know about fish oil.
He also said to never put lotion on at night, especially on your shoulders. You will get it on your face. If you need the lotion on your hands, sleep with cotton gloves on. Also, never eat buttered popcorn. He said you will get it around your mouth and on your chin. Same with ribs, buttered corn-on-the-cob, etc.
He was right about that. All I have to do is drop a little of almost any food or sauce or soup on my chin. Even if I rush to wash it off with acne cleaners and topicals, it's too late. I will break out. I eat ribs with a fork, now.
