For depression, benefits will start when you've been on the optimal dose for you for two weeks. They won't peak until about four to sixteen weeks.

For anxiety, doses usually need to be much higher than for depression. Benefits don't tend to be fully apparent until about 3 or 4 months. Fluoxetine, in particular, has a reputation for increasing anxiety during the first fortnight. This is presumably because it antagonises serotonin-2 receptors, causing release of noradrenaline and dopamine in the cortex (no other SSRI does this).
