If you look online, you'll see that there are a lot of them around $200. Also, go to e-bay and search "light therapy boxes". I saw several for under $150 with a "buy now" option. I don't know whether or not that would be more cost effective than hunting down your own materials.
If you go looking for bulbs to try to fashion your own, remember that the total light output is really the only important factor - that and no harmful UV. The commercial boxes seem to mostly be able to put out 10,000 lux at 14".
