Yes! do I wish I had done that....

If you are lazy like I am -- my grand plans to *THIS* time write down everything every day never pan out-- you can at least make a spreadsheet, list the drug, dates and dosage, and a comment on the overall effect--change in target symptoms, side effects, etc. Also, list the pdoc who prescibed it in case your 14th doctor wants to talk to your 5th....
Obviously more detail is better, but something is better than nothing.

I provided my pdoc such a list, with less detail than I would have liked, but he found it very helpful, and easy to get a quick idea by skimming my spreadsheet....
