Have you ever tried opioids? You could start with tramadol or buprenorphine. If you go with an opioid and its effect starts waining, you can order the CCK antagonist proglumide from overseas and add it. It will counteract the tolerance for one week, then start wearing off. Thus, if you do week on, week of cycles of proglumide, you can keep the tolerance in check.
Have you ever tried isocarboxazid (Marplan)? If your psychiatrist is really cavalier, you can try adding a stimulant to an MAOI, or l-tryptophan (start low, go slow!) to an MAOI, or both. There are ALWAYS options. ;)
