I have done the TCA-stim combo for over a year now (it's the classical treatment for narcolepsy), and yes, it can be rough. Ritalin caused elevated heart rate in conjunction with nortriptyline, and intense orthostatic hypotension. Provigil is much easier on the cardiovascular system than Ritalin, and hasn't forced me to lower the nortriptyline dose (I get seriously DEPRESSED when I go below 75 mg nortriptyline, not to mention assailed by various narcoleptic symptoms, mostly sleep paralysis and hypnagogic hallucinations that are truly unpleasant). Not to say that Provigil doesn't cause its share of side effects. But it is easier to combine, for me, with a TCA.
