Yeah, MAOI's have been considered "the gold standard" for atypical depression, but folks shy away from them, as you know, due to the SE's and the diet. I have not found either to be intolerable.
I initially got them from my GP, who is a bit of a pushover with me. When I realized I needed to think about raising the dose, I called over 15 pdocs to find someone on my plan who was taking new patients. I figured if I could find someone taking new patients, then I'd ask if they'd be willing to prescribe an MAOI without putting me through all the SSRI's again or trying other classes.
I got very lucky in that I got tired of calling, and I called someone back who was out of network but was taking new patients. She has experience with MAOI's and she's been (mostly) great.
I don't recommend that people take the route that I did. I knew what I was asking for and all the precautions more than my GP did. He never said a word about food or drug interactions. You do have to be careful.
