Difficult to say. Vassoconstriction, interaction with other neurotransmitters, direct influence on cardiac muscle via serotonin receptors like 5-ht2b.
Some authors have even gone as far as to suggest that no patient should use SSRI's long term in the advent that they affect the heart in similar ways to drugs such as fenfluramine.
