You're probably experiencing NE overload. Have you ever tried Cymbalta instead (I could never stand Effexor myself)?
The twitching is not surprising...
Internal dermatological conditions should probably be seen by a dermatologist, that sounds more serious than a minor arm rash, at least to me.
