More serious than any long term mutagenesis possibilities, is hyponatremia, which occurred in 2.5% of patients. Low sodium levels are a far more serious and immediate concern and should either be monitored occasionally or at least when hyponatremia sterotypy symptoms appear.
And then there are more rare rash and multiorgan issues.
In general, though it, at least in the short term, is a fairly safe agent and much more tolerated than its predecessor Tegretol. If you don't have too much polypharmacy its probably worth trying if you are going through the laundry list of AEDs and you dont have anything that interacts with 2C19 or 3A4 (e.g. Valium); adjustments would have to be made.
