Drug metabolism is a funny thing.
Do you take stims regularly? Or just on a one off basis? When I started taking Ritalin, I was off my head on 5mg. It was so bad, I stopped taking it. Then, I started taking it again in desperation -- and am now doing fine on 15mg per day. {shrug} I guess I just adjusted to the drug.
I, too, have a history of requiring the highest doses of medications. Mostly ADs, which have always required surprisingly high doses to have any effect. I've been told that for many of these drugs, I'm most likely a poor metabolizer -- my body just isn't very efficient at making use of the drugs, so they have to build up to high levels before they have much effect. For other drugs, though, I may be a very good meetabolizer, thus using up the drugs until they reach such a high concentration in my bloodstream that they can have an effect. I don't understand all that very well -- OK, hardly at all ;-) -- but when it was explained to me it made sense.
So, based on all my ignorance, I suggest that you may be running into a combination of metabolism variation and adjustment phase issues with stims. But, again, what do I know? I'm just a psychiatric patient, after all...
