Oh well, I just sent a second email because the first was returned with a note telling me that Concord is in the process of changing its name to Archimedes Pharma, and their customer service rep has left the company. I managed to locate the new one though. It's no problem - I'm curious to see if the manufacturer will sell direct to a physician, or if they can recommend a pharmacy or something.
