I am no expert but I know much from research, and experience.First I think raising your dose to 90 mg would be a good idea, and give it 2-4 weeks. Hopefully the side efx will be tolerable.
Second I would add a psycho-stimulant, Dexedrine,Provigil, or Concerta (ritalin). This was standard practice (dexedrine) back when. When on Parnate 30-90mgs, we added provigil 100-400mgs. This helped with depression and energy/productivity. This augments the maoi, combats fatigue and weight gain side efx from maoi, and even helps a little with the notorious orthostatic hypotension from maois. These can be very helpful.
Third which is most logical to me, not from experience with maois, is too add lithium. It can have a robust effect when combined with some antidepressants. Once on a regimn of remeron, which wasn't working at all and making me very anxious, we added 300 lithobid and literally a day later I had never felt better in my whole life. Felt normal, depression free, social anxiety non-existent. Unfortunatley it only lasted for 4 days, have some theories now why that happened.
So I think for many with subtheraputic responses to treatments, lithium augmentation of the right drug for the right person, can acheive remission or close to it as opposed to the unfortunate standard in american psychiatry of being satisfied with subtheraputic treatment.
I am considering going on Nardil and most likely augmenting it with Lithium and dexedrine.
