Maybe try adding in a small amount of effexor? I wouldn't let the reuptake inhibitor versus reuptake enhancer thing get in the way. There's a lot more than that going on with both of those meds. Even for euthymic people who don't need tianeptine, it has good benefits. In a 16 month study tianeptine was very effective at preventing relapse. In a long study people who had benefits in the first 14 days had more benefits at 3 months and then experienced even more benefits between month 9 and 12. So just staying the course might be an option too, along with other natural things to help, such as brief bursts of heavy exercise several times a week, heavy emphasis on raw fruits and veggies, lots of purified water, stuff like that.
In an abstract at pubmed there is a case of someone who failed a traditional AD, failed tianeptine, but responded to the combo of them together. So it has been done. No one knows about ther serotonin sydrome risk, so if it were me I would try 37.5mg for a while and feel it out from there.
Maybe a bad idea, maybe a good idea, I don't know. Just a thought. Easiest thing to do without rocking the boat too much.
