Chronic naseau can be depressing in itself. I suffered the same while on paxil... almost 3 months and it never subsided.
Have you tried other ssri's?
I responded well with a combination of zoloft 100mg and remeron 15mg.

If the remeron is working for you at a lower dose, I would be inclined to try other ssri's or snri's. Seems like you need to find something that will work well for you at a steady dose in combination with the remeron.
Lot's of options out there so keep positive.
