Maybe go up to 5mg Lex or 10mg Lex along with the Nortriptyline? That is a true SNRI, except you have the ability to finetune the NE and the 5HT components to your liking.

Check out Dr Gillman's writings at PsychoTropical Research. I think you'll be encouraged to know you were on the right track with the two meds combined, but maybe the Lex was too high, and now it is too low.
