Nardil and Parnate and Clomipramine were, and still are, the kings of the antidepressant world. In 50 years, there still has not been any drug invented that outperforms them.
If you haven't already, another option is to combine serotonin and norepinephrine meds, for example, Zoloft + Nortriptyline. The hype of Cymbalta and Effexor accomplishing that is, well, hype. They have such small action on norepinephrine compared to serotonin. You want a balanced effect on both, which can only be achieved with an MAOI or with two different meds.
