HIPAA today requires that you sign for your own medical records typically.
At certain places where there have been leaks, they go beyond the requirements and it can be very arcane, signing for yourself and the clinic and who knows who else to release your record, but ultimately it is, redundantly, for your security.
Sometimes doctors who are concerned that a patient may analyze a report and read something else out of it are reluctant, but ultimately its your record, and there are very few situations that you aren't allowed to get your complete reports.So yes, ask for them, I do too.
