I would consider seeing a therapist with a Phd. in clinical psychology. This is something that has always been in you. It's a manifestation of your depression and low self-esteem-most likely rooted in whatever neglect/trauma/abuse/lack of love you may have suffered in your childhood.
Also tell your psychiatrist everything you are experiencing so he can hopefully properly diagnose you. You may need a six week trial on an SSRI or other antidepressant to help you get through this and get better.
If you have not taken an SSRI before, you may want to consider avoiding SSRI treatment. I would look into a good ST. John's Wort brand like New Chapter's SC27, Kira, or Perika. The only reason why I take an SSRI now is because I used 2 different ones for several years of my young life. At this point, I'm not sure it would do me much good to try to avoid them. Maybe I should. I think SSRIs can be very effective but I question whether they are the best treatment in the long run. Many suspect that SSRIs can cause irreversible changes.
First, I would try all natural treatments-SJW, Rhodiola, and Fish Oil being at the top of my list. Then, if these don't help(I would take fish oil regardless of whether it significantly impacts your mood) then consider tricyclics or Savella or low dose Lithium. Use SSRIs as a last resort if you can. JMHO and others' I'm sure.
