Severe Mania and Twitching

I think I have tried a lot of different things but nothing is working.
Trileptol, Lithium, Celexa, Ativan, Abilify are my current medications. I have tried a fair few others in the past which I cant remember.
I have hallucinations and hear things, my feet and toes twitch so much I have blisters and my swings last up to 6 months Not sure I can take another 6 months of this. Last week I kicked the dashboard of my truck untill the radio fell out (It was broken anyway)
I work out 3 times a week and work a lot to keep myself distracted and somewhat exhausted but nothing is working.
My Psychiatrist is at a bit of a loss as to what to do with me. I am Bi Polar type A, ADD, Alcoholic. (But I dont drink)
Anyone have any recommendations?
