I guess I agree about the zyprexa increase being a possible culprit. B2c has experience. It mirrors my expeience on abilify. The jump from 2 to 5mg undid any benefit for me.
Pristiq at 100mg didn't work for me, but that was quickly apparent in my case, so if you go that route, you might know pretty quickly. Oddly, given my sensitivities, I had no trouble going back to 50mg of pristiq.
Have you ever tried effexor? It preceeded pristiq as an snri. It worked better for me than pristiq ever did. If you hear horror stories about effexor withdrawal, well, pristiq seemed on par with them. In my experience effexor withdrawal was no worse than pristiq (my experience).
I second altering one medication at a time. If you keep a zyprexa on board, BB's serotax (oh my spelling!) has some merit to consider, but Prozac is it's own very different med from what I gather.
Did you call that pdoc? Maybe you don't want to seem bothersome, but you do have a lot at stake, and that's what they are (supposed to be) there for.
