I've been thinking about why we don't have a post quite like your Internists. Here, hospital consultant physicians are expected to be able to practice general acute medicine. Our consultants are bit like your attending physicians. The training to become a consultant is very long and they learn general medicine as well as their speciality. They don't *just* learn their speciality, they have to be able to deal with a broad range of common medical problems as well.
For example, if you went to hospital with suspected pneumonia you would first go to Accident & Emergency (A&E, like your ER). You would then go to the acute admissions ward. Here, you might be seen by a consultant cardiologist, a consultant gastroenterologist or whichever consultant was covering. All of them would be able to deal with pneumonia. After the AAU, you would go to the appropriate ward for your condition. If you were lucky you might get a private room, but not always!
