1. SSRI antidepressants are usualy 1st choice

2. Gabapentin is a second line choice (only because it is more expensve)

These two can be taken together because they work in different ways

3. Mirtazapine (a tetracyclic antidepressant)

4. MAO Inhibitor antidepressants (treatment of last resort)

I'd start with an SSRI such as Zoloft (25mg per day for a week, then 50mg per day, maybe increase to 100mg per day after 5 or 5 weeks depending on response) and see how you go from there.
