I would try Parnate first (fewer side effects), if that doesnt work, try Nardil, and if that doesnt work, try Nardil + Nortriptyline.
That is assuming that you have already tried
Effexor plus Remeron
Zoloft plus Welbutrin
Zoloft plus Nortriptyline
