I don't recall if you had tried Parnate, but there is usually no weight-gain with it. If you already have, then you are faced with retrying nortriptyline and combating weight-gain. It really is a difficult decision to make. Effexor and Cymbalta did not put weight on me. Weight gain is usually not a problem with Wellbutrin. Perhaps you could go with nortriptyline until a new antidepressant arrives that does not produce weight-gain. I don't think poop-out and failure upon restart are as much of a problem with TCA compared to SSRI. It will be interesting to see what the quality of your response is to nortriptyline if you return to it.
You probably would respond to clomipramine (Anafranil) in the unlikely event that you no longer respond to nortriptyline. In fact, you might be able to switch over to clomipramine from Paxil. Despite its potent SRI properties, clomipamine might produce less weight gain than nortriptyline. However, the anticholinergic side effects it produces are rather pronounced, and sexual side effects sometimes emerge. If it were me, I would go with nortriptyline before trying clomipramine.
I have been able to lose weight while taking nortripytline, so it can be done. Portion control works best.
