I just had this going on recently. For me, I trace it back to stress bringing it on and then my fear and anxiety over it caused it to only increase.
I went to the chiro and he had me doing some stretches and some minor manipulations. Well I think after seeing him it only lasted about 3 weeks.
I was in a lot of pain and distress over this but now it's gone.
Yours sounds like it could be med related. I just wanted to share my story since it was pretty recent.

Diagnosis: depression and anxiety
Current meds: paxil 20mg., desiprimine 20mg., clonazepam .5mg.
Of meds tried, nortriptyline worked the best but caused major weight gain so d/c
