Ketamine treatments around Arizona/ california?

Hello there,

I'm looking for ketamine treatment options around Arizona or California.
I know that JohnLA had it done in California at the university, I don't know exactly where.

Also,
what's the difference between shots and IV infusions? In terms of cost and effectiveness?
