It does happen. Hyperactivity is probably more common with Parnate, and hypomania with Nardil. This is only my impression from having taken both drugs. In both cases, the effects were transient and resolved without further intervention. For me, true, persistent mania didn't develop until after 6 months from the beginning treatment with a combination of Parnate and desipramine.
If mania or hypomania does develop, your choices are to add lithium, add an anticonvulsant, add an antipsychotic, or do nothing. Doing nothing with a more severe mania is untenable. However, some people live with hypomania and function well enough to leave it untreated.
If there is a family history of bipolar disorder, schizoaffective disorder, or schizophrenia, I would suggest watching carefully for the development mania.
