Has clomipramine (Anafranil) been mentioned yet? Although its anticholinergic side effects are unpleasant, it can be potent as both an antidepressant and an anti-OCD.

Combining Luvox with nortriptyline might be an interesting treatment to investigate. Nortriptyline metabolism is not normally affected by Luvox. However, if one is already known to be as slow metabolizer via P450 2D6 enzyme, the dosage of nortriptyline would need to be reduced.
