I'm familiar with the adrafinil hepatoxicity issue.
The idea is that adrafinil is more hepatoxic than modafinil, because adrafinil is a precursor that's converted partly to modafinil and partly to modafinilic acid.
So because there are more conversion steps, one could imagine that more metabolites would accumulate and this could be damaging to the liver. But there actually isn't any hard evidence that adrafinil is more toxic to the liver than modafinil.
