I know someone who has schizoaffective disorder who did not achieve remission with a combination of Abilify ? mg/day and Seroquel 900 mg/day. Her presentation is one of the bipolar types. She would become psychotic with some paranoia; the theme being that there was an intruder in the house. Nothing seemed to work well for her, and she ended up being hospitalized several times. Then, a very inciteful doctor decided to put her on Navan (thiothixene). This cleared up the psychosis well. However, there was some residual depression. She remained on Navane and Sapris (asenapine) was added. This turned on the light for her. It cleared up her thinking and was energizing. The net effect is that she achieved full remission and lost all of the Abilify + Seroquel weight gain. My doctor thinks that Saphris is particularly good for treating cases where paranoia exists. Saphris does have antidepressant properties.

If you are not too keen to move to Navane at this juncture, then I would consider switching from Seroquel to Saphris. Zyprexa is often the second best of the atypical neuroleptic antipsychotics; Clozaril being the first. Of course, this is not universally true. It is amazing, but a real pain in the butt, that two people will respond differently to the same drug. I think it may be too early to go to Clozaril and deal with weekly or biweekly blood tests to screen for agranulocytosis (neutrophils).
