Emsam + insurance

Hi, has anyone had Emsam be "non formulary" (i.e. not covered) by their insurance and successfully filed an "prior authorisation"? I really want to try it but don't want to pay ~ $1,700
/month...
FTR I've tried every med and treatment in the book, if that counts any.
