like abilify, the idea is that instead of blocking D2 receptors, like most antipsychotics/neuroleptics, it "occupies" the receptor, blocking normal dopamine transmission and also offering some ability to stimulate the same receptor. I'm not a science person, but...

this seems to translate--with Abilify, at least--into less EPS, less dysphoria, less...typical neuroleptic experiences, basically. And yet...it still gets the job done, for most people, in Bipolar I, mania, Schizophrenia, Schizoaffective, etc.

with abilify, some animal studies have shown that there's less pronounced brain changes from long term treatment than with, say, zyprexa or haldol. i think animal studies hint at a lower risk of tardive dyskinesia, too, but...

-all- neuroleptics can cause: neuroleptic malignancy syndrome, EPS, dysphoria, TD, akathisia, possibly tardive dementia (I think that's usually associated with tardive dyskinesia and/or tardive akathisia, but not always...).

there's the controversial concept of "tardive psychosis," which means that administering a neuroleptic can cause people who had never previously experienced psychosis to experience psychosis, probably because of changes to brain structure. along the same lines, there's an emerging awareness that while drugs suppress key symptoms, there seem to be cases where the problems get worse, not better. and so...

one has "treatment resistant depression" emerging from antidepressant usage; severe anxiety from benzodiazepine use; and tardive psychosis from neuroleptics. but...

even with all that, there are people/patients who clearly do far better -on- psych drugs than off psych drugs. personally, i think the big, big problem is that so many people have been exposed to so many drugs for problems that might not have been treated with drugs (or with anything, at all) 20, 30 years ago. :-(
