Someone I know who has been treatment-resistant did well for awhile using intranasal ketamine taken about every 5 days. She was visibly improved. She has since decided to try Trintellix (vortioxetine). I don't why she was unhappy with ketamine.
I tried intranasal ketamine every day for two weeks. I felt nothing, but I question the quality of the compounding pharmacy and the dosage being too low. If you don't respond to intranasal ketamine, you can still go on to respond to IV treatment.
If you haven't tried Trintellix yet, it might be worth taking a look at. It is looked upon favorably by Andrew Nierenberg at Harvard / Mass General for treatment-resistance. I did not react well to it, but I wouldn't generalize that to everyone else. I have had bad reactions to other drugs as well.
