celexa and ultram

Posted by jane on April 2, 2000, at 11:01:26

my pharmacist recently warned me and the danger of interactions of celexa and ultram (the possibilities of seizures) I was surprised because I have been on both for a year. Does anybody have any experience (positive or negative) with taking this combination.
Thanks-jane

Jane - Your pharmacist may have extrapolated some of this information from another SSRI - Prozac. Prozac is thought to have caused serotonin syndrome in a person taking Ultram. This is possible because Prozac is a potent inhibitor of the same enzyme system that metabolizes Ultram (CYP-2D6 - for the techies). This means that the Prozac will increase the level of Ultram in your bloodstream. Since Ultram can cause seizures, increased blood levels may increase the risk of these seizures. This scenario is when taking Prozac. Celexa also inhibits CYP-2D6, but to such a small extent that it probably could be considered insignificant.

Since you have been taking this combination for a year, the chances of developing seizures on this combination is minimal. Do bring it up the next time you see your doctor, but I think this pharmacist was being overly cautious.

*Caveat* - Ultram is not sold in Canada, and I have no personal experience with it. The above information is book knowledge. Get a second opinion before trusting my info. Sincerely - Cam W.

>Hi Jane,

I've been taking Ultram in various dosages for about a year, right now I take 2 or 3 tablets per day for chronic pain. When the psychiatrist changed my antidepressant from Effexor to nortriptylene he warned me that the combination could cause seizures; that he had a patient in the hospital who had suffered a seizure from such a combination. So he asked me to reduce the Ultram as I increased the nortrip. Well, that kind of scared me but not enough to cut back. I asked the neurologist about it a couple of weeks ago and he said he has never had a problem with Ultram in combination with antidepressants and told me not to worry. I think I would have had a seizure by now anyway if it was going to happen. Just Ultram by itself causes seizures in some people.
