LIQUID V/s SOLID form, WHY different efficacy?

Hi to every!
At the beginning apologize for my awful English!
I am on a neuroleptic not far different from amisulpride. I'am using it in low dosage for a mild deprssion with negative simpthoms.
At the beginning i start using the liquid form (drops) with success and no side effects: mood improve, more energy, more alertness...etc.
After 4 months i passed to the pill (solid) form, and i noticed (using same dosage, of course) reduction in rigth effects and mild drowsy and somnolence.
I thought that it was "all in my mind" and i stay on that formulation for months.
After months i passed again to the liquid formulation and noticed another time that the right effects appear again and dizzy disapear.
To be sure that it was not my illusion, passed again to the pill form with a reduction of right effects again and more somnolence.
Now i'm sure that it is not my immagination, but WHY can it be possible? I use same substance, same brand and same dosage, only formulation is differt!
I know that liquid formulation is more FAST to enter in circuit in your blood, but not other differences.
Could anyone give me an answer on it, or anyone could report similar strange experience on drugs?

Hi Mike. The answer to you question is one word, digestion.

Here's the longer answer. When you use a drug in liquid form, usually you put some in your mouth, and hold it there. The membranes lining your mouth are not chemical-proof. They leak. Dissolved drugs can pass right through those membranes, and enter your circulation directly. In fact, some of that drug will immediately encounter the blood/brain barrier, permitting rapid absorption into the brain.

In contrast, when you swallow a pill, it encounters stomach acid, then membranes with very large amounts of digestive enzymes right in them, then the liver. And the liver is full of enzymes, some the same as the digestive membranes, but others with much enhanced destructive power. Digestion is designed to break molecules down. And that happens to drugs.

So, the oral route probably gives you a higher "real" dose of the drug, to begin with (none broken down), delivers it right beside the brain, and goes to work right away.

You might find that holding a pill in your mouth until it dissolves has a similar effect to the liquid form of the drug.

Sincerely i have not thought about that possibility. I've only supposed that there was something in the different adjunctive compounds present in the drug. But we know that they should be inactive at all.
I'm completely in accordance with you and think you're right, but let me say that (and is only an inpression) even if you let dissolve the pill in the mounth, there is not the same of haveing it in the liquid form.....i think there is, however, something different as if liquid were metabolized in a different way or bind neurotrasmitters differently.

Yes, I am on L-sulpiride (levosulpiride, brand name Levopraid). It is a purified fraction of sulpiride. Sulpiride is composed by two stereo-isomers (l- and r- sulpiride). The first is the only active, r- is supposed to be inactive and that produce any side-effects.
So you can say that l-supiride is twice as potent as sulpiride with supposed less side effects.
I'm currently on 75 mg/d (three times a day 25). A greater quantity seems to produce to me more somnolence.
I tryed Amisulpride. The first 2 days it was great! I could say it produced a cocaine-like effect. But in the following days it faded and somnolence and anxiety came over caused probably by the Tolerance.
Never found out a schedule to make Amisulpride work continuosly.

Levopraid is available in Belgium and Italy, right?

How long have you been on Levopraid? Any tolerance?

Yes i'm from Italy but i think it is also available in any other country of suthern Europe, isn't true?

I've been on and off it for about four years.
Unlikely to Amisulpride, L-sulpiride seems not to produce tolerance. I think so, cause it binds weaker dopamine receptors. But the way, i always use to take it for 4-8 weeks and then stop for 1-2 week to let the system clear.

I don't know. Levosulpiride seems to have been studied/researched mainly in Italy.
